I have been having an ongoing issue with autopilot disabling itself at what seems to be completely random times.
It has proven to be fatal only a small number of times (usually when docking) but it happens fairly frequently. It cannot be reproduced on demand, but it happens at least once per play session. It can happen when autopiloting through a gate, following another ship or even docking.
This is very similar to a problem I had with X3:Reunion where the autopilot would disengage when docking, generally ending with my ship smashing into the internal docking port of trading stations.
I have tried switching my keyboard and mouse with spares, making sure my xbox 360 controller is unplugged and checked for updated drivers. I have also verified the game quite a few times and even reinstalled it a number of times since this problem first appeared.
I can only assume it is some type of configuration error as I have not seen this problem reported anywhere yet.

Have you checked to make sure the hotkey commands aren't conflicting with something else? The normal hotkey for it is Shift-A, and the docking command Shift-D acts similarly in certain circumstances.

This occurs when I have my hands off the controls, not when I press the hotkey.
For example when checking the controls a minute ago I pressed Shift + D on a station that was approx. 25km away. When it started the approach to the docking lane (after turning to face the lights) it just said "Autopilot Off" and cancels the docking maneuver like I just tried to move the ship manually but I wasn't touching the keyboard or mouse.
The keys I have set are as follows:
From BP:
CAG Admin: F11
Turbo Boost: Shift + B
Turret On/Off: h
Vanilla Commands:
SETA: j and Mouse 5 (back button on mouse)
Target Nearest Enemy: Shift + T and Mouse 4 (forward button on mouse)
All other controls are unchanged from default or not bound as normal.

Do you have any other peripherals plugged into your USB ports? Also check for any other programs running in the background. Anything that could be issuing commands via the USB ports, or PS2 ports if you're using those style keyboard or mouse. Try turning off any automatic updates, anti-virus software, etc., and see if the problem goes away. It does appear that something external may be interfering with the game.

Change to a conventional mouse and see if the problem goes away. At least then you will know the culprit, if not the button. The signal that is interfering may not be related to a printable character; it could be a game-unidentifiable signal from a programmable macro button for example. It could even be that the mouse is not correctly calibrated and has a very slight cursor drift.
One other thing occurs to me. Make sure that your ship is not in a group or wing and/or that it does not have other ships set to follow/protect it as that might be causing the Autopilot lapses when the followers have issues.
