The Invisible Trader.
I docked at a station and walked to the Trader's Corner (didn't use the transporter). I was greeted with only a floating icon. Waited maybe around a minute, the model didn't appear. Reloading the game fixed it, the model loaded in a few seconds in.

Hesheit should probably start a performance career instead of hanging around in the docks
The model was doing that every time that character was speaking. Once the talk was done, their head returned to a more... agreeable position. Again, reloading fixed it.
