[BUG] Ships teleport randomly with new orders

By far the biggest issue that I have had that is preventing me from really playing is this:

I have 15 ships right now,
2 freighters on Trade>galaxy
1 freighter on Autotrade
3 Miners on automine
9 fighters/scouts

The freighters and miners seem to work fine and have no issues, however, the fighters and scouts have some huge issues.
First, and I don't think these ones are bugs, they just don't work:
1. When giving ships the explore command they sit around for hours doing almost nothing (I fixed this with the autoexplore mod which works pretty well). I am pretty sure this is just how it's meant to work, but it sucks.
2. When giving ships an attack command, they may or may not go after the enemy, and when they do they never actually kill anything. They fly in circle around the enemy and sometimes (SOMETIMES) fire their weapons. They usually just end up dying. This may be because my pilots are all 0 star pilots and they just suck, but i've also found no way to increase their skill level. I have over 36 hours in my play through and all my pilots are still 0 stars and have never gained any skill.

Second, the worst actual bug that I've had related to ships:
Ships will teleport or fly to the ship that I am in and then set their orders to attack me.
This only happens to fighters and scouts. It seems to be completely random. Sometimes it happens shortly after I give them orders and sometimes it happens when they're sitting around with no orders. Sometimes they can be on orders for an hours or sometimes it doesn't happen to them at all. Very intermittent, but it happens very frequently. I'll give orders to a bunch of ships and before I know it they're all in my sector firing on me instead of doing what i told them to do. To be clear, when I say teleport i don't mean the teleport game mechanic built into the game, which i don't have. I don't even know how to research yet. I mean ships will physically teleport from whatever sector they are in, all the way across the universe to like 20km away from me and then they will immediately get orders to attack the ship i'm flying.

Anyone know how to fix this? anyone else having this issue?
