I am confused that many ay they start a new game with 1.3
Does that mean I have to otherwise some changes do not apply the devs did? Or can I continue my old Save and get all the effects of the changes form the updates?
1.3 restart required?

You can continue your save game. There is no need to restart, unless the devs specifically mention it in the patch notes which they haven't.
