Mods: Orange cursor mod (happens with it disabled as well)
The game video/rendering hitches every few seconds.
Updated AMD drivers, a few days before noticing the issue.
I used DDU to uninstall and install the version I had prior, but I'm not sure if the problem was present in the last 2 versions of AMD Drivers.... Haven't played in about 1.5 months.

Hi, you have a huge list of sound devices and drivers on the go at the same time. Disable any you don't use in Control panel, Device Manager, Sound/Audio Devices (right-click on a device in the listing for a disable/enable pop-up). Make sure that whatever sound drivers you keep are fully updated to latest WHQL version.
You don't have Windows Error Reporting enabled so there are no clues there in your DxDiag.
Make sure that you don't have anything running in the background while playing that periodically calls home to check for updates/contacts or to report in. That might explain a regular brief loss of game focus. You could try giving the game better cpu processing priority if you know how to do that in case that helps.

I've gone ahead and cleaned up the audio devices to bare minimum and will perform further testing.
