This is my first playthrough and it would seem the Terran CovOps informant Delilah has disappeared from my universe. I was running through a handful of story missions, strayed away from this storyline for a little bit and after destroying Saman Red's ship for step 3, I went to complete step 4 and she and the ship she was on are nowhere to be found.

DxDiag would be virtually irrelevant, as this persists across a high-end desktop and a lower/medium laptop.
I did as much digging as I could independently, all I found was that this might be caused by doing the Yaki questline in parallel to the Terran CovOps? One post mentioned that if the ship Delilah resides on despawns then she's simply gone from the universe forever and there's no way to get her back other than to revert to a save before this has happened. I have no such save, there was probably 20-30 hours between me starting this questline and the moment I realized she had Houdini'd out of existence.

We can't diagnose what's happened from a screenshot (for the record, I can't even see that as Imgur is not available here!) and without a savegame there is nothing for us to look into. Unfortunately we have no way of confirming that the mods you're using, or have used in the past, are not the cause of any problems you're having, so even with a savegame we can't really investigate.

Try restarting the mission without using the teleport.
