I've never experienced this nor can I recreate it. Headtracking has worked perfectly as expected for me every time I've used it.
I use Opentrack -> Freetrack for lots of headtracking games.
When I went to play around with hacked VR for X4 that is what I used.
To do that I did a clean install of the latest version of opentrack with all default settings.
SteamVR (and Virtual Desktop) was used as the Input and freetrack 2.0 enhanced (With TrackIR, disable freetrack selected).

https://youtube.com/shorts/IPM4PXsYIis?feature=share (note in this video I was not testing this specifically, I just happened to have it, I was in VR with a quest so the headset is weighted forward, that plus opentrack's default center created the "weight" upon returning to center, I was testing the "center" just like in any headtracking setup so I am on purpose returning to center also I shook my head a little sometimes to test it)
Later, I saw people talking about the problem so I went testing again. It works as intended in TrackIR, freetrack, or Both Enabled modes in opentrack as expected.
This time I used a Logitech C270 Webcam and the Neuralnet tracker. The only thing I changed was some deadzones due to the erraticness of the tracking which was visible in Opentrack.
Again that behaved as expected.

I have HOTAS controls bound including an analog axis to look and that seems to disable itself when headtracking is enabled.
What's more Is I'm pretty sure I changed 0 settings in X4 and that headtracking has just worked for me.
I think there is something else going on on the client side of these setups. Misconfiguring of opentrack's centering modes?
